Director of Nursing - Nurse Practitioner (Full Time)

  • Global Transitional Care (GTC) is the first third-party specialty group provider organization dedicated to providing comprehensive transitional care. Our mission is to enhance the care continuum and provide personalized clinical oversight for each individual patient.

    GTC helps avoid complications and readmission to the hospital by helping to manage all aspects of care from inpatient stay to home for 30 days from date of discharge. Through a transitional care provider team, patients and their families will have access to a healthcare provider that is familiar to them and their case, 24 hours a day, 7 days a week.

    GTC’s goal in transitional care is to give the newly discharged patient and their healthcare team continuity of medical and post discharge care, further reducing adverse events, unnecessary return visits to the hospital or emergency room, and the knowledge to personally manage their own health condition.

    Under the supervision of the Medical Director and Chief Nursing Officer, the Director of Nursing (DON) assumes authority, responsibility, and accountability for the delivery of transitional care nursing services through Global Transitional Care (GTC). In collaboration with the executive team, the DON allocates department resources in an efficient and economic manner to enable each patient to attain or maintain the highest practical physical, mental, and psychosocial well-being. Collaborates with other facilities, medical professionals, consultants, and organizations, including government agencies and advocacy groups, to develop, support and coordinate resident care, related administrative functions, and to represent the interests of the organization. The DON also leads and supports care programs and research, and assists in marketing, business development, outreach, education, quality improvement, and other initiatives as needed.

    Major Responsibilities:

    • Develops, maintains, and implements nursing policies and procedures that conform to current standards of nursing practice, facility philosophy, and operational policies while maintaining compliance with state and federal laws and regulations.

    • Communicates and interprets policies and procedures to nursing staff, and monitors staff practices and implementation.

    • Participates in all patient care decisions, and performs AP-TCN duties* on an ongoing basis.

    • Participates in daily or weekly management team meetings to discuss patient status, census changes, personnel, or resident complaints or concerns.

    • Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate services and staff coverage in all regions.

    • Demonstrates knowledge of and application of the Transitional Care Model, and proactively monitors and implements systems to support the model.

    • Exercises overall supervision of patient assessments and care plans.

    • Oversees or is responsible for the Transitional Care certification programs, competency testing, and all related records

    • Develops, maintains, and reviews weekly report from every region to monitor and ensure timely, effective communication to significant changes in condition, and care plans.

    • Collaborates with physicians, facilities, community agencies, and institutions to improve the quality of services and to resolve identified problems.

    • Oversees nursing schedules to assure they meet patient needs and regulatory and budgetary standards.

    • Participates in the recruitment and selection of nursing personnel and assures sufficient staff are hired.

    • Oversees and supervises development and delivery of in-service education to equip nursing staff with sufficient knowledge and skills to provide compassionate, quality care and respect for patient rights.

    • Proactively develops positive employee relations, incentives, and recognition programs. Promotes teamwork, mutual respect, and effective communication.

    • Performs rounds to observe care and to interview staff, patients, families/caregivers or other interested parties.

    • Proactively develops procedures and incentives to promote workplace safety and safe work practices.

    • Coordinates and/or develops on-going QA activities for nursing services to monitor nursing compliance with standards and regulatory requirements through rounds, interviews, and record reviews. Compiles summary of findings for the Quality Assurance committee.

    • Helps the Administrator prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc. Reviews and reinforces important standards previously cited.

    • Participates in the preparation of the Plan of Correction response to an inspection survey, and implements any follow-up QA required for any nursing allegations.

    • Communicates directly with patients, families, medical staff, nursing staff, interdisciplinary team members, and regional managers to coordinate care and services, promote participation of care plans, and maintain a high quality of care and life for patients.

    • Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from patients or families about nursing services.

    • Monitors service incidents and complaints as needed to identify those defined as unusual occurrences by State policy and promptly reports such occurrences to Administrator/Executive Director for appropriate action.

    • Monitors complaint reports as needed for allegations of potential abuse or neglect, or the loss or misappropriation of patient property, and participates in these investigations.

    • Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness.

    • Provides training, oversight and supervision of assigned AP-TCNs and TCNs, conducting new-hire orientations, performance evaluations, reviewing patient satisfaction surveys, and reporting back to the Chief Nursing Officer and Medical Director

    Conditions of Employment:

    • Authorization to work in the US

    • Graduate of an accredited Nurse Practitioner Program

    • Current Nurse Practitioner or Certified Nurse Specialist certification

    • Five (5) or more years as an RN, preferably in acute care or case management setting, current CA RN license

    Preferred Qualifications:

    • Public Health and/or Case Management certification

    • Community Education Experience
