Legacy Healthcare in Fairmont, WV is seeking an experienced Director of Nursing (DON) to lead nursing operations in our skilled nursing facility. The DON will advance resident care, ensure regulatory compliance, and mentor nursing staff.

You will supervise RNs, LPNs, and CNAs, oversee QAPI initiatives, manage staffing, EMR usage, and collaborate with physicians and administrators to maintain high-quality, compliant care.

Qualifications

• Current West Virginia RN license required.
• 3–5 years nursing leadership in SNF/long-term care.
• Thorough knowledge of CMS/state regulations.
• Experience with EMR systems.
• Strong leadership and communication skills.

Responsibilities

• Provide leadership to the nursing team (RNs, LPNs, CNAs).
• Oversee daily clinical operations for resident care and safety.
• Ensure CMS and state regulatory compliance.
• Develop and monitor nursing policies and procedures.
• Manage staffing, scheduling, and workforce planning.

Job Title: Director of Nursing (DON)
Location: Fairmont, WV
Facility Type: Skilled Nursing Facility (SNF)

Position Summary: We are seeking an experienced and compassionate Director of Nursing (DON) to lead and oversee all nursing operations within our skilled nursing facility in Fairmont, West Virginia. The DON will ensure the highest standards of patient care, regulatory compliance, and staff development while fostering a supportive and collaborative environment for residents and team members.

Key Responsibilities:

• Provide leadership and direction to the nursing team, including RNs, LPNs, and CNAs

• Oversee daily clinical operations to ensure quality resident care and safety

• Ensure compliance with federal, state, and local regulations, including CMS guidelines

• Develop, implement, and monitor nursing policies and procedures

• Manage staffing, scheduling, and workforce planning

• Lead quality assurance and performance improvement (QAPI) initiatives

• Collaborate with physicians, administrators, and interdisciplinary teams

• Oversee infection control, medication management, and clinical documentation

• Mentor and develop nursing staff through training and performance evaluations

• Participate in surveys, audits, and accreditation processes

Qualifications:

• Current RN license in West Virginia (or ability to obtain)

• Minimum of 3–5 years of nursing leadership experience, preferably in a skilled nursing or long-term care setting

• Strong knowledge of regulatory standards (CMS, state regulations)

• Proven leadership, organizational, and communication skills

• Experience with electronic medical records (EMR) systems

• Ability to manage multiple priorities in a fast-paced environment

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN) or higher

• Previous experience as a Director of Nursing or Assistant Director of Nursing

• Certification in gerontological nursing or nursing administration (a plus)
