The Director of Home and Community Based Services is responsible for planning, organizing, developing, and coordinating the overall operation of the Home Health Agency and provide oversight of Concierge Care Management component of Plymouth Harbor Health Services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as directed by the Vice President of Health Services, to assure the highest degree of quality resident care.

Essential Functions

  • Establish and coordinate in conjunction with the Director of Nursing of Home Care the implementation of written policies and procedures in cooperation with the Sr. Vice President of Health Services.
  • Establish intra and inter-departmental rapport.
  • Ensure that all personnel, residents, patients, visitors, and the general public follow established policies and procedures.
  • Interpret facility’s policies and procedures to personnel, residents, patients, family members, visitors, etc. as indicated.
  • Review and/or revise policies and procedures at least annually and/or as indicated to assure compliance with current regulations.
  • Ensure that residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, and property and civil rights are established and maintained.
  • Maintain a public relations program that services the best interests of the residents, facility and community.
  • Maintain a good working relationship with the medical profession, and other health related facilities and organizations.
  • Recruit, select and orient competent departmental personnel.
  • Meet with department directors on an as-needed basis. Conduct and/or participate in in-service classes and supervisory level training programs.
  • Advise the Sr. Vice President of Health Services as to the number and qualifications of nursing personnel required to meet the health needs of residents in the Home Health Agency.
  • Oversee billing for home care services.
  • Oversee payroll for Health Services/Home Care.
  • Review monthly financial statements and interpret information pertinent to the Home Health Agency in cooperation with the Sr. Vice President of Health Services.
  • Cooperate with the Sr. Vice President of Health Services in the management of the departmental budgets.
  • Keep abreast of economic conditions and make adjustments that will assure the continued ability to provide quality nursing care.
  • Represent the Home Health Agency at designated meetings.
  • Represent Plymouth Harbor in dealings with outside agencies, including governmental agencies and home health agencies.
  • Delegate authority, responsibility, and accountability to other responsible persons, as appropriate.
  • Attend and participate in workshops, seminars, etc. to keep abreast of current changes in the health care field, as well as to maintain a professional status.
  • Request equipment and supplies for the department as needed.
  • Complete team member performance appraisals in a timely manner.
  • Submit wage increase recommendations to the Sr. Vice President of Health Services in a timely manner.
  • Make written and oral reports and recommendations to appropriate individuals or committees concerning the operation of the Home Health Agency.
  • Assess, evaluate, and implement appropriate recommendations from the facility’s committees, e.g. Pharmaceutical Services Committee and Infection Control Committee, etc.
  • Assist in the development and implementation of an in-service program that will enhance the assurance of quality resident/patient care.
  • Organize and standardize methods by which department objectives will be accomplished.
  • Attend and participate in workshops, seminars, and mandatory in-services as approved or required.
  • Maintain work area, equipment and supplies in a clean, sanitary and organized manner. Ensure that work areas are neat and confidential materials are properly stored before leaving area on breaks, end of work day, etc.
  • Work safely; complying at all times with Plymouth Harbor safety standards.
  • Call 555 in the event of an emergency.
  • Discharge properly such other responsibilities and duties as the Sr. Vice President of Health Services may direct.

  • Licensed physician, physician assistant, or registered nurse licensed to practice in this state or an individual having at least 1 year of supervisory or administrative experience in home health care or in a facility licensed under chapter 395, under part II of this chapter, or under part I of chapter 429.
  • Must possess knowledge of laws, regulations, and guidelines pertaining to Assisted Living and Home Health Agency.
