Directors of Health Services (DHS) are Registered Nurses in our Community-Based Care settings who provide leadership, direction, and manage the healthcare services we provide. Here, we are a family striving to enhance the lives of every person we serve!
The primary purpose of the Director of Health Services is to: plan, organize, develop, and direct the overall operation of our Health Service Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility. This role is also to be carried out as directed by the Executive Director and Regional Nurse Consultant to ensure the highest quality of care is maintained at all times.
Essential Duties and Job Responsibilities:
- Delegate tasks according to Oregon Board of Nursing Rules and Regulations, ensuring proper documentation and training.
- Develop and maintain good rapport with all parties involved in the care plan to ensure team effort in developing comprehensive care plans.
- Report known or suspected incidents of fraud, waste, and abuse.
- Review the 24-hour report and health monitoring, making recommendations as needed.
- Assess the use of supportive devices with restraining qualities per established standards.
- Plan, develop, organize, implement, evaluate, and direct the health service department and its programs in accordance with Oregon Administrative Regulations and OSHA guidelines.
- Coordinate care with outside and offsite providers.
- Participate in Continuous Quality Improvement and develop improvement plans for identified issues.
- Participate in facility surveys and inspections by authorized government agencies.
- Coordinate services among the Interdisciplinary Team.
- Ensure all personnel are aware of and refer to the resident’s care plan prior to providing care.
- Implement and adhere to company policies and guidelines.
- Review incident reports and assist with investigations.
- Complete required assessments and monitor resident conditions as per policy.
- Participate in all compliance training programs and care conferences.
Requirements and Qualifications:
- Nursing Degree (LPN or RN) from an accredited institution.
- Minimum of three (3) years supervisory experience in a hospital, nursing care facility, or related healthcare setting.
- At least six (6) months experience in rehabilitative and restorative nursing practices.
- Current, unencumbered RN license in Oregon or willingness to obtain one within 6-12 months.
- Knowledge of nursing practices, laws, and regulations pertaining to nursing care facilities.
- Leadership and supervisory skills, with ability to work harmoniously with staff.
- Ability to relate to and work with ill, disabled, elderly, and emotionally upset or hostile individuals.
- Willingness to adopt new methods and principles into nursing practices.
- Fluent in English (reading, writing, speaking).
- Active CPR/BLS Certification.
