Terrace Home Health is seeking a Director of Clinical Services – Home Health (RN) to lead clinical operations in Independence, Missouri. This hands-on RN leadership role focuses on improving patient outcomes, regulatory compliance, and staff development across the agency.

You'll partner with the Administrator to strengthen clinical quality, ensure accurate documentation, and drive QAPI initiatives while coaching a dedicated clinical team to deliver exceptional care in patients' homes.

Terrace Home Health is seeking an experienced Director of Clinical Services – Home Health (RN) to lead clinical operations in Independence, Missouri. This leadership role is ideal for a registered nurse with home health clinical management experience, strong OASIS knowledge, and a passion for improving patient outcomes, regulatory compliance, and team performance.

You’ll work closely with the Administrator to strengthen clinical quality, develop staff, improve documentation accuracy, and support the delivery of exceptional home health care.

Key Responsibilities

• Clinical Leadership: Oversee daily home health clinical operations, clinical staff performance, and quality of patient care.
• OASIS and Documentation: Ensure accurate OASIS assessments, timely clinical documentation, and compliance with applicable documentation requirements.
• Quality Improvement: Monitor CMS quality measures, patient outcomes, 5-Star performance, and Quality Assurance and Performance Improvement (QAPI) initiatives.
• Regulatory Compliance: Support compliance with Medicare requirements, CMS Conditions of Participation, and applicable federal and Missouri regulations.
• Case Management: Oversee care coordination, patient assessments, individualized plans of care, and interdisciplinary communication.
• Staff Development: Recruit, mentor, coach, and develop clinical staff while promoting accountability, engagement, and retention.
• Clinical Performance: Identify performance gaps, implement corrective actions, and drive continuous improvement in clinical processes.
• Leadership Collaboration: Partner with the Administrator to support agency goals, survey readiness, patient satisfaction, and operational effectiveness.

Required Qualifications

• Current Registered Nurse (RN) license in good standing.
• Previous home health clinical leadership, supervisory, or management experience.
• Strong hands-on knowledge of OASIS assessments and home health documentation.
• Understanding of Medicare home health regulations, CMS quality measures, and clinical compliance requirements.
• Knowledge of patient care standards, case management, and plan-of-care oversight.
• Strong clinical judgment, communication, organization, and leadership skills.
• Ability to coach and develop employees, manage competing priorities, and work effectively in a fast‑paced environment.

Preferred Qualifications

• Experience improving CMS quality measures and Home Health 5‑Star ratings.
• Experience with QAPI, regulatory surveys, and corrective action plans.
• Hospice experience.
• Experience mentoring clinical teams and improving staff retention.
