Qualifications

• Graduation from an accredited Nurse Practitioner program.
• Five years of experience as a practicing Nurse Practitioner, two in administrative/managerial capacity.
• Possession of a New York NP license during appointment and employment.

Responsibilities

• Oversee medical and clinical staff in consultation with the medical director.
• Ensure antibiotic stewardship and regulatory compliance; audit notes and documentation.
• Lead clinical programs focused on wound care, infection control, psychotropic stewardship, and fall prevention.
• Provide direction to personnel delivering medical and health services.
• Attend professional meetings and oversee policy updates.
• Consult for patients and medical providers when necessary.
• Lead QAPI programs and participate in department and facility meetings.
• Participate as adjunct in the admissions process.

Job description

This is professional administrative and clinical work providing supervision of professional, medical and nursing staff to meet the needs of residents within a nursing home setting. The work is performed in accordance with standards and guidelines prescribed by the medical profession, New York State Department of Health, Centers for Medicare and Medicaid Services (CMS), and the Centers for Disease Control Prevention. This employee works under the general direction of the executive director/nursing home administrator and consults with the medical director to assure the medical needs are met for each resident.This position is responsible for all medical-related policies/procedures, quality improvement related to clinical/medical quality measures and employee health programs. Does related work as required.

Responsibilities/Typical Work Activities:

• In consultation with the medical director, provides oversight to the medical and clinical staff.
• Oversight includes, but is not limited to, antibiotic stewardship, auditing compliance with regulatory and monthly notes, adequacy of documentation, fluency with facility’s electronic medical record (currently PointClickCare), hospital transfer activity and data collection.
• Manages clinical programs focused on wound management, infection prevention and control, psychotropic stewardship and fall prevention;
• Confers with and provides direction to the various personnel charged with the responsibility for providing professional, medical and health-related services;
• Attends professional meetings and provides policy and procedure oversight/updates;
• Provides consultation for any patient or medical provider if necessary;
• Leads Quality Assurance and Performance Improvement (QAPI) programs and attends QAPI, department head and other facility administrative meetings;
• Serves as adjunct in the admissions process.

Full Performance, Knowledge, Skills, Abilities and Personal Characteristics:

• Thorough and up-to-date knowledge of the principles and practices of general medicine;
• Thorough and up-to-date knowledge of federal and state regulations as they apply to skilled nursing facilities;
• Thorough knowledge of clinical standards of practice related to patients in a nursing home setting;
• Ability to diagnose and treat a variety of diseases and injuries;
• Ability to provide direction and supervision to employees;
• Ability to work with electronic medical records;
• Ability to read and interpret medical charts and records;
• Physical condition commensurate with the demands of the position.

Minimum Qualifications (either):

Graduation from an accredited Nurse Practitioner education program registered and approved by the New York State Department of Education and five (5) years of experience as a practicing Nurse Practitioner, two (2) years of which shall have been in an administrative and managerial capacity.

Special Requirement:

Possession of a license to practice as a Nurse Practitioner in the State of New York at the time of appointment and throughout employment.
