Position Responsibilities
  • Champion the culture built on our mission, vision, and values.
  • Build a culture of engaged Care Navigators to become the Employer of Choice for remote care navigators.
  • Ensure excellent patient experiences and outcomes resulting from strong relationships built between patients and their Care Navigators who are well-equipped to empower patients with chronic conditions.
  • Manage the day-to-day operations of the Clinical Department.
  • Achieve operational excellence in the clinical department. Highlight management process gaps or improvement opportunities and implement solutions based on a continuous improvement mindset.
  • Lead critical projects from design through execution.
  • In cross-functional collaboration, ensure adequate staffing to meet patient needs and business objectives.
  • Identify, track, and analyze data that will inform KPI achievement. Prepare reports and presentations for senior leadership.
  • Build strong relationships with our partners as we provide remote clinical staff services to support their patients.
  • Optimize Care Navigator and clinical department workflows to achieve efficiency and effectiveness.
  • Closely collaborate with the Q / A and Compliance department to ensure adherence to all legal regulations, policies, procedures, and quality assurance initiatives.
Experience & Skills
Required Experience and Qualifications
  • Proven track record of leading teams to achieve established department and organizational goals.
  • Advanced computer skills, including use of Google Suite, Salesforce, EHR systems, and spreadsheet management.
  • Strong data analysis with a keen eye to see and understand how to leverage KPI data to drive desired patient, department, and business outcomes.
  • Demonstrated ability to understand complex issues and data to identify trends, gaps, and solutions to business challenges.
  • Successfully identify and communicate data to front-line users that is actionable and inspiring.
  • Strong problem-solving and decision-making skills.
  • Strong communication skills and ability to connect with a variety of audiences including executives, and external partners, LPNs, physicians, new leaders.
  • Bachelor of Nursing (BSN) or combination of experience and education
  • Master’s degree in Nursing or related field preferred.
  • Ability to work within a diverse and cross-functional team.
  • Effective time management skills and ability to manage competing priorities.
  • Must have 5+ years of progressive leadership experience, preferably in Remote Patient Monitoring, Chronic Care Management, or TeleHealth.
  • Ability to work autonomously in a remote-first environment.
Preferred Experience and Qualifications
  • Operations and / or Project Management Experience, PMP certification is a plus.
