• Experience with program development or quality improvement initiatives.

Responsibilities

• Lead and mentor a team of 10–12 care management professionals, providing guidance and development.
• Establish clinical standards for assessments, care plans, and clinical decision-making.
• Oversee the client experience from initial conversations through ongoing services and follow-up.
• Champion a whole-person approach addressing medical, emotional, social, and daily needs.
• Facilitate collaborative case discussions to navigate complex client situations.
• Design and pilot innovative care models addressing real client needs.
