Job summary

CareOne is hiring a Case Manager at our Greensburg, PA facility to coordinate patient care within a Managed Care framework. The role focuses on optimizing Length of Stay, preventing avoidable readmissions, and ensuring seamless discharge planning.

The ideal candidate holds an RN license, an associate degree (Bachelor's preferred), and at least one year of LTC case management experience. Strong communication and organizational skills are essential for success.

Qualifications

• Must have an Associate's Degree; Bachelor's degree preferred.
• Must hold a State Certified RN license.
• Must have a valid driver’s license and reliable transportation.
• Minimum of one year experience as a Case Manager, LTC preferred.
• Excellent communication, be well organized and able to multi-task.

Responsibilities

• Oversees care for patients insured by Managed Care to ensure appropriate Length of Stay and coordinated discharges.
• Provides ongoing communication to insurance case managers and primary care providers.
• Coordinates services from multiple disciplines in the facility.
• Completes initial patient and family assessments for home care needs.
• Regularly re-evaluates nursing needs and updates care plans.
• Collaborates with healthcare team and patient family to ensure continuous, well-coordinated care.
• Initiates preventative and palliative nursing procedures as needed.
• Communicates changes in patient needs with the physician.

THE REHABILITATION & NURSING CENTER AT GREATER PITTSBURGH

The Case Manager responsibilities will include, but are not limited to:

• Oversees the care provided to patients insured by Managed Care organizations, ensuring they receive efficient care with appropriate Length of Stay, limiting re-hospitalizations and well-coordinated discharges.
• Provides ongoing communication information to insurance case managers and primary care providers.
• Works to coordinate services provided by the various disciplines in the facility.
• Completes an initial assessment of patient and family to determine home care needs
• Regularly re-evaluates the patients nursing needs & use assessment data to determine care plan needs
• Collaborates with the healthcare team, patient and patient's family, and helps direct the work of the healthcare team to provide care that is continuous and well-coordinated
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding the patient's needs and reports any changes.

Position Requirements

• Must have an Associate's Degree, Bachelor's preferred
