Job summary

HCA Houston Healthcare Medical Center is seeking a Director of Case Management to lead and coordinate department activities, drive performance, and ensure high-quality patient care. You will oversee discharge planning, care coordination, and resource utilization while partnering with senior leaders to achieve organizational goals.

This role requires an RN or equivalent clinical background, a Bachelor’s degree, and a track record of 2+ years in leadership within hospital case management.

Qualifications

• Bachelors Degree required.
• Master's degree preferred in Nursing, Health Admin, or Business Admin.
• RN or LCSW credential preferred; travel not required.
• 3 years in acute care hospital case management; 2+ years leadership.

Responsibilities

• Directs and evaluates departmental operations and staffing to meet objectives.
• Allocates resources to meet productivity and quality goals.
• Oversight of CM core functions and practice.
• Ensures adherence to care coordination and discharge planning processes.
• Collaborates with CFO and Division CM Leader to achieve goals.
• Assesses performance and implements improvement initiatives.
• Ensures policy compliance and SOP adherence.
• Establishes relationships with stakeholders including CMO, CNO and Legal.
• Performs other duties as assigned.
• Upholds Code of Conduct and Mission and Values.
