Qualifications

• Master of Science Degree in Nursing required, or a Bachelor of Science Degree in Nursing and a Master of Science Degree in healthcare, finance, or education required.
• Minimum four years nursing experience with competencies in case management, clinical nursing, leadership, administration, or education.

Responsibilities

• Directs and oversees day-to-day case management functions across three hospital sites.
• Develops and maintains a comprehensive, fiscally sound case management program.
• Collaborates with the AVP Care Coordination to ensure cohesive patient-centered care from admission to discharge.
• Maintains relationships with payers and oversees onsite reviewers.
