The Director of Case Management will lead the hospital's Case Management department and oversee daily operations, staffing, interdisciplinary care planning, utilization review, discharge planning, patient advocacy, and overall quality of case management services.
- Manage the day-to-day operations and human resource functions of the Case Management department
- Oversee interdisciplinary plans of care and the discharge planning process
- Evaluate the effectiveness and appropriateness of services with focus on census management, patient outcomes, and key care indicators
- Provide leadership, guidance, training, and support to Case Managers and other department staff
- Coach staff on caseload management and interpretation of regulations, policies, procedures, and operational objectives
- Review departmental operations to maintain quality and consistency with organizational standards
- Advocate for patients and families throughout care delivery and transition planning
- Develop relationships with insurance companies, self-insured employers, case management firms, and healthcare networks
QUALIFICATIONS
- Minimum of three years of hospital-based Case Management experience
- Experience must include both Utilization Review and Discharge Planning
- Must be qualified to independently complete assessments within the scope of the candidate's professional discipline
- Active professional licensure required when mandated by the discipline and state
- RN candidates must hold a BSN and active RN licensure
- Other eligible healthcare professionals must hold at least a bachelor's degree; graduate degree preferred
- Current CCM or ACM certification preferred
