Qualifications

• 3 – 5 years’ experience in a hospital setting as a case manager with utilization review experience
• Certification as a Case Manager preferred
• Licensed as an RN in the state of Texas, BCLS.

Responsibilities

• Plan, direct, implement and monitor hospital-wide care management efforts.
• Coordinate programs across departments to ensure appropriate care and admissions.
• Perform concurrent reviews to justify extended stays.
• Refer cases not meeting guidelines to Utilization Review Physician Advisor.
• Assist in patient facilitation via bed management and multi-disciplinary activities.
• Develop hospital-wide Care Management strategies and ensure regulatory compliance.
• Ensure resource allocation, discharge planning and social work processes.
