Job summary

Qualifications

• Licensed clinician in your state with post-graduate social work experience.
• Minimum 5 years in a case management position and strong data skills.
• Knowledge of standard utilization review processes and criteria.
• Preferred CCM or eligibility within 1 year; BCLS and related credentials valued.

Responsibilities

• Lead and supervise case management staff and discharge planners.
• Develop staff and systems to operate a comprehensive program.
• Coordinate services using continuous quality improvement tools.
• Assess needs, design services, and integrate care aligned with hospital mission.
