Salary: $115,000 - $130,000 per year

We are seeking an experienced Case Management Director to oversee the daily operations and strategic direction of the Case Management department within an inpatient healthcare setting. This leadership role is responsible for ensuring effective care coordination, discharge planning, utilization review, and patient advocacy while driving positive outcomes for patients, families, and the organization.

Key Responsibilities
  • Lead and manage the day-to-day operations of the Case Management department
  • Provide leadership, coaching, and development for Case Managers and departmental staff
  • Oversee interdisciplinary care planning and discharge planning processes
  • Ensure appropriate utilization of healthcare services and resources
  • Monitor key performance metrics related to patient outcomes, length of stay, and census management
  • Serve as a patient and family advocate to ensure high-quality, patient-centered care
  • Interpret and implement organizational policies, regulatory requirements, and best practices
  • Maintain collaborative relationships with payers, insurance companies, employers, and healthcare partners
  • Drive quality improvement initiatives and operational efficiencies
  • Foster a culture of accountability, collaboration, and excellence
Qualifications
  • Active clinical licensure, if required by professional discipline and state regulations
  • CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification preferred
  • Ability to independently perform patient assessments within scope of practice
  • For nursing candidates: Bachelor's Degree in Nursing (BSN) and active RN license required
  • For other healthcare professionals: Bachelor's Degree required; Master's Degree preferred
  • Minimum of 3 years of hospital-based Case Management experience
  • Previous leadership or management experience preferred
  • Strong communication, relationship-building, and problem-solving skills
