Job summary

Diversicare is seeking a dedicated Social Worker to support residents in post‑acute care settings. You will interview residents and families, assess needs, and help plan care and discharge steps. The role focuses on advocacy, coordination, and ensuring access to services within the facility.

Experience in nursing homes, hospice, home health, or hospital settings is preferred. This full‑time position emphasizes teamwork and compassionate care within a supportive environment.

Qualifications

• One year of supervised social work experience in a health care setting.
• Experience in healthcare setting – nursing home, hospice, home health, hospital.
• Ability to work with a variety of people in different settings.
• Able to make decisions based on evaluations of case histories.

Responsibilities

• Assist in pre-admission and admission process with customers; aid clients in adjusting to nursing home placement.
• Interview residents and families to obtain relevant past and present adjustments, behavior and lifestyle.
• Document progress and updates plans of care at the interdisciplinary care conference.
• Arrange transportation, make appointments and liaise between departments, families and outside agencies.
• Assist in handling and accepting placement through counseling and group work.
• Ascertain potential and develop discharge plans when clients are admitted.
• Arrange for post discharge services and follow‑up care.
• Assess emotional, financial, spiritual, educational and social needs of residents and ensure those needs are met.
• Act as resident/family advocate and ensure the resident understands rights.
• Organize and conduct Family Counsel Meetings and maintain minutes.
• Maintain effective communication with residents, families, healthcare professionals and staff.
