Job summary

Diversicare is seeking a dedicated care coordination professional to lead daily care coordination meetings, oversee the restorative nursing program, and ensure seamless communication with patients, families, and insurers across our Mississippi facilities.

You will guide care plans, collaborate with clinical teams, monitor quality measures, and help navigate regulatory requirements to support safe, compassionate care for residents.

Qualifications

• Degree in nursing, therapy or related clinical field and a current license.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience leading teams and projects; strong strategic planning abilities.
• Excellent verbal, written and interpersonal communication skills.
• Knowledge of PDPM requirements of insurers.

Responsibilities

• Facilitates daily care coordination meetings to align resources with patient needs.
• Leads care management discussions and coordinates with the clinical team.
• Oversees restorative nursing program integration with activity department.
• Communicates changes in condition and discharge plans to patients and families.
• Manages reviews with HIM and insurers, ensuring timely documentation.
• Coordinates discharge planning and post-discharge communications.
