Job summary

Central Peninsula Hospital seeks an experienced Director of Care Coordination to lead our care coordination team and improve quality of care, patient satisfaction, and health outcomes.

This role is open to a qualified RN or Master’s-level Social Worker with leadership and case management experience. It also offers relocation assistance and participation in the Director-level Incentive Compensation Plan, with up to 10% annual incentive potential.

Qualifications

• Bachelor's Degree in Nursing or Master's in Social Work required.
• Alaska Licensure as a Registered Nurse or LMSW required.
• 2 years of leadership experience required.
• 2 years of case management or utilization review experience required.

Responsibilities

• Oversee the clinical operations of the care coordination team.
• Instill management practices to drive teamwork and timely achievement of goals.
• Develop and integrate strategies to improve quality of care, patient satisfaction, and outcomes.
• Ensure cost-effective patient care for patients and families.
