Diversicare is seeking a Director of Care Coordination to lead and optimize care resource alignment for patients and residents. The role emphasizes coordinating the restorative nursing program, guiding daily meetings, and ensuring timely communication with families regarding changes in condition and discharge planning.

The ideal candidate has clinical leadership experience, strong communication skills, and knowledge of PDPM and regulatory requirements.

Qualifications

• Must hold a current license in respective clinical field.
• Expertise in healthcare reimbursement and utilization management.
• Experience leading teams and managing projects.
• Strong ability to develop short and long-term plans.

Responsabilités

• Facilitate daily care coordination meetings.
• Coordinate restorative nursing and activity integration.
• Lead care plans with clinical team input.
• Oversee discharge planning and post-discharge communication.
• Monitor quality measures with the care coordination team.
• Ensure timely Medicare and Managed Care documentation per guidelines.
• Review ADRs to ensure requests are met.
