Job summary

Diversicare Healthcare Services, LLC invites a strategic Director of Care Coordination to lead our care resource alignment for patients and residents, ensuring optimal outcomes and service quality.

You will oversee the restorative nursing program, coordinate daily care coordination meetings, manage discharge planning, and communicate with patients' families about changes in condition and discharge plans, while guiding the clinical team to meet PDPM and regulatory requirements and driving

Qualifications

• Degree in nursing, therapy, or related clinical field; current license.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience providing strategic leadership, managing teams and leading projects.
• Ability to formulate and implement short and long-term strategic plans.
• Strong verbal, written, and interpersonal skills.
• Proven ability to lead a team to achieve outcomes.
• Knowledge of PDPM requirements and regulatory requirements.
• Previous case management experience preferred.

Responsibilities

• Facilitate daily care coordination meetings to align care resources with patient needs and benefits.
• Participate in and facilitate Engage/72 HR Admission meetings as part of the care management system.
• Manage the restorative nursing program, integrating it with the activity department.
• Ensure timely communication with patients and families regarding changes in condition and discharge planning.
• Collaborate with the clinical team to develop and execute care plans.
• Coordinate discharge planning and post-discharge communication, including home health and DME.
• Monitor Quality Measures alongside the care coordination team.
• Serve as a skilled care documentation specialist, ensuring accurate and timely completion of Medicare and Managed Care regulatory guidelines.
• Review Additional Documentation Requests (ADRs) to ensure all documents meet the request.
