Job summary

Lifepoint Health invites an experienced clinician to join Rainer Springs as an Outpatient Therapist with an emphasis on assessment and care coordination. You will engage with physicians, families, and staff to determine appropriate levels of care and ensure smooth admission processes across multiple services.

The role requires a Master’s in social work or counseling (RN licensure accepted per state) and strong clinical assessment skills, with emphasis on CMS/state regulations and compliant

Qualifications

• Master's degree in social work, counseling or related field required or licensed per state guidelines.
• RN licensure may be used per state practice guidelines.
• Clinical assessment skills and ability to work with psychiatry and geriatric patients.

Responsibilities

• Directs and performs screening of potential patients for admission and initiates assessment.
• Assesses level of care needs and coordinates with staff and physicians.
• Communicates with external reviewers and manages required reviews under HIPAA.
• Prepares morning census and referral reports for leadership.
• Liaises with families, transportation, and other facilities to organize admissions.
• Ensures accurate transfer of patient information to attending physicians.
