Job summary

Central City Health in Detroit, MI seeks a Director, Case Management and Social Health to lead an integrated case management workforce across behavioral health, housing, and social support services. You will oversee care coordination and service navigation to promote continuity of care and reduce fragmented delivery.

The role manages caseload and program fidelity while collaborating with clinical, housing, and organizational leadership to address social determinants of health and improve patient

Qualifications

• BLS certified required.
• Master's degree in social work, public health, or related field; Michigan LMSW or equivalent licensure preferred.
• Minimum 5 years leadership experience spanning case management, care coordination, or social services, ideally across behavioral health and housing/SDOH contexts.
• Familiarity with IPS Supported Employment fidelity, HUD housing program requirements, and 42 CFR Part 2 confidentiality boundaries for SUD-adjacent case management work.

Responsibilities

• Own the unified case management model: ensure members with cross-cutting needs get one assigned case manager, not multiple uncoordinated handoffs.
• Directly manage the Case Management Pool, the Outreach & Community Health team and the two Housing Specialists cross-training into full case management roles.
• Coordinate with clinical supervision/licensure matters for case management staff and maintain administrative authority.
• Coordinate with Senior Team Lead and Administrative Manager of Housing to maintain HUD and housing-program fidelity.
• Own the IPS program via its dedicated team, ensuring fidelity to the IPS model.
• Develop outreach/community engagement initiatives in coordination with leadership.
• Establish and monitor acuity-weighted caseload standards across the unified pool.
