Job summary

Advocate Health in the United States seeks a senior leader to drive community health strategy across local markets. You will partner with facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact colleagues to align priorities with broader organizational goals.

Responsibilities include leading CHNA cycles, developing programs addressing social drivers of health, and establishing metrics to measure impact and guide improvements.

Qualifications

• Bachelor's degree in Public Health, Public Administration, Community Planning, Health Administration or other relevant field.
• 7 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas.
• Strong analytical and writing skills.

Responsibilities

• Partner with facility leaders and health partners to align local health priorities with organizational goals.
• Lead Community Health Needs Assessment cycles and related strategies and governance responsibilities.
• Develop and refine community health programs addressing social drivers of health.
• Establish performance measures, evaluation approaches, and reporting processes.
• Source and advance local community projects for potential low-interest loan investment.
• Support community engagement and build relationships with local stakeholders.
• Oversee data entry, documentation, and validation for reporting and standards.
• Translate data and progress into clear recommendations for leaders and partners.
• Coordinate cross-functional collaboration to implement initiatives.
