Job summary

IMPaCT Care in Pittsburgh, PA seeks a Director of CHW Services to lead a team of one Manager and ten Community Health Workers, supporting Highmark Health initiatives and community-based programs in the Pittsburgh area.

You will shape program strategy, oversee day-to-day operations, and drive high-quality patient support and outcomes. A graduate degree and 5+ years in healthcare with leadership are required; bilingual candidates are a plus.

Qualifications

• Graduate degree in Social Work, Public Health, Population Health, or Healthcare Administration.
• 5 years of healthcare experience with 2+ years in a leadership role overseeing operations, initiatives, and budgets.
• Experience recruiting, managing, and developing high-performing staff.
• Exceptional presentation, interpersonal, and writing skills.

Responsibilities

• Oversee a team structure of one Manager and 10 CHWs, providing leadership and accountability at both levels.
• Build your team by partnering with community-based organizations to recruit and hire CHWs and administrative staff.
• Work on-site to oversee day-to-day programmatic and personnel operations, supervising staff in alignment with program goals.
• Integrate CHWs into community and clinical settings and ensure high-quality patient support.
• Deliver and report high performance on quality outcomes and program satisfaction for all individuals receiving CHW support.
• Achieve high patient and staff satisfaction to promote retention and growth of the program.
• Build IMPaCT's presence in the Pittsburgh area through partnerships and strategic engagement activities.
• Ensure high-quality delivery of client services — including planning, hiring, training, and programmatic support — to support Highmark Health's CHW program locally.
