Job summary

HCA Healthcare is seeking a Director of Case Management at Mission Hospital McDowell to lead the case management team and ensure seamless patient discharge planning across departments. Qualifications

• Bachelor's degree required.
• Master's degree preferred in Nursing, Health Administration, or Business Administration.

Responsibilities

• Directs and evaluates departmental operations, including case management model, staffing, information technology use, onboarding, and staff competencies.
• Allocates resources to effectively staff department and meet productivity and quality goals.
• Responsible for oversight of CM core functions and practice.
• Ensures adherence to care coordination and discharge planning processes with timeliness and proper documentation.
• Coordinates with Facility CFO and Division CM Leader to achieve goals and expectations.
• Assesses and improves department performance by evaluating processes and data, and implementing improvements.
• Ensures compliance with policies and SOPs.
• Establishes working relationships with stakeholders including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
• Performs other duties as assigned.
• Practices and adheres to the Code of Conduct and Mission and Value Statement.
