Job summary

Saint Mary’s Health Network, a Prime Healthcare member in Reno, seeks a Director of Case Management to lead a multidisciplinary team and oversee a comprehensive program for patient care, discharge planning, and utilization review.

You will mentor case managers and social workers, develop policies, and drive quality improvement to enhance patient outcomes and hospital efficiency in Northern Nevada.

Qualifications

• Licensed clinician in your state.
• LCSW credential is preferred; grandfathered prior to 2015 acceptable.
• Minimum 5 years’ post graduate experience in Social Work for LCSW.
• Minimum 5 years’ experience in a Case Management position.
• Analytical ability for problem identification and data evaluation.
• Experience with basic to intermediate computer skills.

Responsibilities

• Director of Case Management is responsible for the development of staff and systems to effectively operate a comprehensive Case Management Program.
• Provides leadership and supervision to case managers, social workers and discharge planners, utilization review coordinators and technicians.
• Assesses needs, plans and designs services aligned with hospital mission and patient/family needs.
• Coordinates services using continuous quality improvement tools.
