Job summary

SCO Family of Services is seeking a Direct Service Worker for a part-time, on-site role in Bayport, NY. You will provide close supervision per individual treatment plans and assist in program meetings and documentation.

Qualifications include a high school diploma or GED and a valid NYS driver’s license; prior experience with developmental disabilities is preferred. SCIP and AMAP training are required or pursued.

• Experience working with people with developmental disabilities preferred.
• Ability to complete SCIP and AMAP training.

Responsibilities

• Provides close supervision for individuals per the treatment plan.
• Participates in program planning meetings and signs annual/semi-annual plans.
• Maintains possessions neatly and documents needs to management.
• Transports individuals safely and maintains vehicle logs and condition.
