Job summary

Responsibilities

• Assist individuals with ADLs, including grooming, cooking, cleaning and hygiene.
• Support community participation and recreational activities.
• Accompany individuals to appointments, shopping, or outings.
• Encourage skill-building for independence and life skills.
• Implement and document individualized support plans and ISP outcomes.
• Teach skills such as cooking, budgeting and personal safety.
• Support communication and self-advocacy efforts.
• Report incidents or changes to the supervisor promptly.
