Job summary

Sevita is seeking a dedicated Direct Support Professional in Indiana to provide non-exempt, hourly direct support in residential or periodic settings. You will assist individuals with daily living activities, health maintenance, and community integration, delivering services per individual plans.

The role requires strong communication, confidentiality, and teamwork, with emphasis on safety and proper documentation. Applicants should meet basic education requirements and be at least 18 years old.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; review records and logs to stay abreast of changes in service plans; maintain confidentiality.
• Maintain healthy and professional relationships with individuals, families, guardians and case managers; implement Sevita Customer Service Standards.
• Report any instance of alleged abuse or neglect and medical, behavioral or other incidents according to policy.
