Qualifications

• Direct support with individuals served.
• Maintain confidentiality and accurate records.

Responsibilities

• Deliver support per individual service plans, including socialization and daily living activities.
• Record progress and maintain logs; protect confidentiality.
• Assist with personal care, housekeeping, transportation, and community activities.
• Communicate with supervisor about health, safety, and incidents.
