Job summary

Journey Found, Inc. is seeking a Direct Support Specialist to assist individuals in our group homes in Manchester, CT. Responsibilities include helping with daily living activities and coordinating with clinical staff to support care plans.

Experience in behavioral management is valued; training is provided. Requires High School diploma and driving record with a valid license. Equal opportunity employer and a strong commitment to safety.

Qualifications

• High School Diploma or equivalent required.
• 3+ years of driving with a valid license preferred.
• Behavioral management experience or training helpful.
• Strong oral and written communication skills required.
• PMT, CPR, FA and MAP training may be required.
• Must be free from communicable disease.

Responsibilities

• Assist individuals with activities of daily living (dressing, cooking, cleaning, bathing, toileting).
• Transport clients to appointments and activities.
• Organize community outings and supervise activities.
• Administer medications as required and document accordingly.
• Coordinate with nurses, clinicians and supervisors to ensure care plans.
