Provides support for individuals and/or groups of persons with disabilities in the community. Assists the person(s) receiving services in identifying and communicating their requests and needs for services and supports. Implements services and supports to address the person’s goals and desired outcomes as identified through each person’s Centered Plan. Seeks to understand individuals in the context of their personal history, social and family networks, and their hopes and dreams for the future. Relies on direction and supervision to accomplish the job. Requires a High School Diploma or GED (preferred in TN), at least 18 years of age, and a valid driver’s license.
Starting Pay Rate: $15.00/hr
Schedule: Shifts and schedules may vary by location and typically include a variety of shifts covering all days of the week, Monday through Sunday.
