Job summary

We Do Life...Together seeks an entry-level Direct Support Professional in Naugatuck, CT. This role involves direct care and oversight for individuals, ensuring their needs align with individual plans and daily living requirements.

Responsibilities include assisting with medical/behavioral appointments, transportation, medication administration (Med Certification required), and maintaining plans in accordance with state guidelines.

Responsibilities

  • Ensure the needs of the individuals are met consistently and in accordance with the individuals’ plan.
  • Maintain and update individuals’ plans as needed in accordance with state and agency requirements.
  • Responsible for daily housekeeping tasks and daily upkeep of the individual’s home and/or work environment.
  • Provide safe transportation of individual(s). Ensure agency vehicles are maintained and required documentation is completed timely.
  • Administer medication and implement delegated responsibilities ordered by licensed medical personnel (Med Certification Required).
  • Communicate regularly to management team on individual’s performance, successes, and issues.
  • Actively participate on the Interdisciplinary Team focused on development/implementation of the Individual Plan.
  • Partner with other DSPs and administrative team members to share expertise and insights.
  • Identify and participate in projects/initiatives that contribute to long-term excellence and individuals’ growth.
  • Develop understanding of the workings of each department that interact with individuals.
  • This position is considered essential staff and is critical towards the care of our individuals.
  • Physical demands include standing, walking, and sitting; occasional lifting up to 25 pounds; may require working outside.

Job Responsibilities

Required Trainings and Certifications
  • DDS Med Certification (within first 6 months of employment)
  • CPR
