Job summary

WellLife Network in New York seeks a compassionate Direct Support Professional to assist adults with developmental disabilities in daily living, health care, and community participation. You’ll support personal care, nutrition, safety, and mobility, and help individuals achieve goals in home and community settings.

Collaborate with healthcare teams, document care in electronic records, advocate for clients’ rights, and promote independence in a respectful, person-centered approach.

Qualifications

• High School Diploma or equivalent required.
• Fluent in English for client communication.
• Valid New York State driver’s license with at least 2 years of driving experience.

Responsibilities

• Provide hands-on personal care and daily living support.
• Assist with meal preparation, chores, and safe home environment.
• Administer medications as directed and accompany medical appointments.
• Assist mobility and transfers using mobility aids as needed.
• Plan and participate in recreational and community activities.
• Maintain accurate records in electronic systems and progress reports.
