Qualifications

• Minimum 18 years of age or older.

• Good driving record and valid driver’s license may be required.

Responsibilities

• Assist with activities of daily living and health and safety procedures.
• Support individuals in community and home settings.
• Assist with person-centered plans and outcomes.
• Document progress and data for reporting.
