Job summary

Seven Hills Foundation seeks a Direct Support Professional III - LPN to deliver direct medical support and education to individuals in assigned programs, advocating for medical needs and aiming for optimal health. The role covers on-site clinical tasks, documentation in EHR, and collaboration with DDS standards.

Hours and on-site responsibilities are defined by supervisors. The position requires MA LPN/RN licensure, 1–3 years of related experience, and experience with MR/DDS.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current Massachusetts license.
• 1–3 years of related experience.
• One year working with individuals with Department of Developmental Services (MR/DDS).
• High school diploma/GED/HiSET required

Responsibilities

• Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant SHCS staff.
• Assist and transport individuals to medical appointments in accordance with DDS standards.
• Document all nursing interventions, medical updates, and care changes in Electronic Health Records (EHR).
• Maintain and review medication and treatment orders; monitor for side effects and report concerns to appropriate personnel.
• Support ISP implementation by understanding and monitoring relevant care components (e.g., behavior plans, hygiene, self-medication).
• Train staff on medication policies and procedures.
• Ensure health records are accurate and compliant with DDS regulations.
• Communicate medical updates and treatment plans to residential and day program staff.

Direct Support Professional III - LPN

Pay: $40.00/hour base rate; $41.00/hour Cape Cod rate

Schedule: Sunday 9am-9pm, Monday through Wednesday 7am-3pm; Mandatory staff meeting the 1st Thursday of every month at 9:30am

The Direct Support Professional III - LPN is a direct service position providing individual medical support and education to individuals in assigned programs who advocate for all medical needs of assigned program participants focusing on optimal health for all individuals. Hours assigned by supervisor; on call responsibilities.

What You'll Do

• Train staff on medication policies and procedures
• Ensure health records are accurate and compliant with DDS regulations
• Communicate medical updates and treatment plans to residential and day program staff
• Maintain safety standards and monitor for compliance across settings
• Collaborate with Health Care Director or program RN for supervision and clinical guidance
• Participate in staff meetings and mandatory in-services
• Uphold agency policies and maintain professionalism, confidentiality, and respect at all times
• Foster a supportive and cooperative team environment
• Perform additional duties as assigned by the Health Care Director or RN
