Job summary

Momentum is hiring a Direct Support Professional II (DSP II) to provide support to adults with disabilities. You will implement ISPs to promote independence and ensure a safe home environment within Momentum’s Independent and Community Living Services program.

The position includes essential responsibilities such as coaching daily living skills, assisting with daily tasks, and coordinating transportation and community resources, with travel across Los Angeles and Orange Counties.

Qualifications

• Must be 18 years of age or older.
• California driver license, liability insurance, and reliable transportation (mileage reimbursed).
• High school diploma or equivalent; required.
• Must have reliable phone access (monthly stipend).
• Minimum of one year experience with people with developmental or other disabilities (preferred).
• Must read/write/communicate effectively in English.
• Pass post-offer pre-employment screening including background check and drug screening.
• Ability to lift up to 50 lbs and transfer individuals.
• Use of personal vehicle for assignments; traveling across service areas (LA and Orange Counties).

Responsibilities

• Provides individualized coaching in daily living skills.
• Assists with daily needs/tasks within persons served homes.
• Provides education and support in using community resources.
• Maintains accurate records in ISP-related logs.
• Supports daily activity schedule including meals, sanitary conditions, budgeting, and transportation.
• Prompts and assists with medication management.
• Teaches adaptive living skills and monitors adherence.
• Assists with personal care and medical/dental appointments.
• Encourages participation in social activities to improve social skills.
• Trains participants to access public transportation and benefits (Social Security, SSI, IHSS, Section 8).
• Assists with transfers and mobility as needed.

As a Direct Support Professional II (DSP II), you will be responsible for the implementation of Individual Service Plans (ISP) that will lead to or continue to promote independence and personal skills, well assisting in providing a safe and secure home environment for persons supported in Momentum’s Independent and Community Living Services program.

DSP II Essential Responsibilities:

• Provides individualized coaching in daily living skills
• Assists with daily needs/tasks within persons supported home
• Provides education and support in the use of community resources
• Maintains accurate and current records in individuals’ communication logs regarding information relevant to Individual Service Plans (ISP)
• Supports and assists persons supported in following their daily activity schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks, budgeting, ledgers, and transportation, as necessary
• Prompts and assists persons supported in medication management
• Teaches adaptive living skills and monitors the extent to which they are followed
• Assists participants in all aspects of personal care, including hygiene and medical/dental appointments
• Assists individuals in attending social activities to further social skills
• Trains program participants to access public transportation and assist persons supported in accessing benefits such as Social Security, SSI, IHSS and Section 8 housing
• Assists persons supported in weight bearing and non-weight bearing transfers

DSP II Qualifications:

• Must be 18 years of age or older
• Current and valid California driver license, liability insurance, and reliable means of transportation; required
• High school diploma or equivalent; required
• Must have reliable phone/access to phone (monthly stipend)
• Minimum of one (1) year experience working with people with developmental and/or other disabilities; preferred
• Must be able to read, write, comprehend, and communicate effectively in English
• Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
• Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
• Use of personal vehicle for assignments throughout service areas not normally assigned; highly preferred
• Must be available to travel throughout Los Angeles and Orange Counties, as needed (mileage reimbursed)
