Job summary

Constructive Partnerships Unlimited is seeking staff to support participants through conferences, treatment plan development, and daily living activities in a structured environment. You will work with a clinical team to implement treatment plans and maintain thorough records, ensuring safety and adherence to procedures.

Responsibilities include assisting with daily living activities, coordinating transportation, monitoring health and diet, and participating in training programs.

Qualifications

• Completion of required medication administration course.
• Completion of Cerebral Palsy orientation training program.
• Compliance with all applicable laws, rules, regulations, policies and procedures including corporate compliance training at least annually.

Responsibilities

• Attend participant annual, semi-annual, and special conferences as needed.
• Assist habilitation counselors and clinical team in evaluating participants and participate in developing the participant’s individualized treatment plan.
• Implement participant’s treatment plans by following prescribed methodologies.
• Maintain records and charts by entering required information (goal/data sheets, behavioral data sheets, inclusion forms, etc.).
• Assist participants with activities of daily living (toileting, showering, grooming, meals, dressing).
• Monitor participant’s health and report concerns to nursing staff.
• Escort participants to and from transportation and ensure safety on/off vehicles.
• Monitor dietary concerns and report to nursing staff.
• Accompany participants to medical appointments as assigned.
• Participate in special programs, inclusionary and volunteer opportunities.
• Attend in-services and specialized training as assigned.
• Prepare and set up classrooms for day’s activities; maintain cleanliness and safety.
• Perform all other duties as assigned.

Education

Completion of OMRDD Approved Medication Administration Course

Completion of Cerebral Palsy orientation training program
