Job summary

SRVS in Clarksville, TN is seeking a Direct Support Professional (DSP) for a full-time evening shift (Monday–Friday, 3:00 PM–11:00 PM). The role pays $15.50 per hour and focuses on daily living support, personal care, meals, and independence-building under PCSP guidelines.

You will document care, monitor health and safety, and help participants participate in community activities while maintaining accurate records. A compassionate, dependable team player is essential.

Qualifications

• High School Diploma or GED required.
• Must be 21 years of age or older.
• Must pass required physical and pre-employment drug/alcohol screening.
• Must pass required criminal background check.
• Ability to exercise independent judgment in crisis situations.
• Ability to complete and maintain required State and agency training and certifications.
• Must maintain a current driver's license and required vehicle documentation when transporting individuals.
• Flexibility to work overtime as required.
• Ability to perform essential job functions with or without reasonable accommodation.

Responsibilities

• Provide direct support with daily living, personal care, meals, grooming, and other individual needs.
• Help individuals build independence and work toward goals outlined in their PCSP.
• Support community, social, recreational, and other scheduled activities.
• Follow and implement individual behavioral, therapy, and support plans.
• Administer and document medications as assigned and in accordance with agency procedures.
• Monitor health, safety, behavior, and well-being and report changes or concerns appropriately.
• Maintain accurate daily notes, progress documentation, incident reports, medication records, and other required records.
• Transport individuals safely when required and maintain applicable driver's license, insurance, and vehicle documentation.
• Assist in maintaining a clean, safe, and supportive home environment.
• Complete required agency and State training, attend scheduled meetings, and maintain required certifications.
• Follow infection control, safety, emergency, incident reporting, and agency procedures.
• Maintain accountability for medications, personal belongings, funds, vehicles, supplies, and other daily essentials.
