Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. This role supports 4–6 individuals in residential settings, or up to 10 in periodic programs, in West Virginia.

You will assist with daily living, health maintenance, medication administration, and community integration, reporting outcomes and maintaining confidentiality as part of a mission-driven team.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.
• Valid driver’s license in good standing; car registration and vehicle insurance if providing transportation.
• All state-required trainings and certifications completed in mandated timeframes.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping, and other activities of daily living.
• Accurately document progress and activity; review records and logs to stay informed about changes in service plans; maintain confidentiality.
• Make cost-effective and appropriate purchases within the budget of the individual served and according to their choice.
• Maintain healthy and professional relationships with individuals, families, guardians and case managers; greet visitors and uphold Sevita Customer Service Standards.
• Report any instances of abuse or neglect and other incidents per Sevita policy; maintain documentation of medical and behavioral events.
• Respect rights and confidentiality; practice universal precautions; assist individuals in exercising their rights.
• Attend orientation and ongoing training; participate in monthly staff meetings.
• Work scheduled hours; arrive on time; communicate clearly when unable to work; may fill in for other shifts if needed.
• Ensure workplace safety and report hazards; follow safety protocols and participate in safety drills.
• May transport individuals into the community; drive safely and adhere to local laws; maintain safety equipment.
