Job summary

STRiVE is seeking a Direct Support Professional in Grand Junction, CO, to provide community supports for Adventurous Minds Program participants with high physical, behavioral, and medical needs. Duties include assisting with daily living activities, personal care, and social integration in community settings.

The role emphasizes safety, goal attainment, and person-centered planning, with on-site work schedule Mon-Fri and background check clearance required.

Qualifications

• Must be over 21 for driving purposes and have valid driver's license.
• All positions at Strive are contingent on passing a background check.

Responsibilities

• As a Direct Support Professional (DSP), you will provide compassionate care helping support individuals with disabilities live fuller lives. Working on helping them reach goals with activities of daily living, social independence, and being a part of their community.
• Listen attentively and take general interest in assisting people with enjoying meals, maintain personal hygiene, and learning their routines.
• Support individuals in assisting them with making and maintaining relationships with friends, family, and their loved ones. Assisting with making phone calls, setting up get-togethers, and regular interactions. Must be able to drive and have valid drivers license.
• Assist with medical needs, including monitoring health, administering medications, and daily documentation. Strive provides training with QMAP Qualification, CPR, and Vital Signs for maintaining individuals' health and well-being.
• Assist with transporting and getting individuals out into the community, for work and recreational experiences.
• Collaborate with Co‑Workers and supervisor in maintaining a positive and safe program/work environment.
