Job summary

Qualifications

• High School Diploma or GED is required.
• Must have valid driver’s license and a clean driving record.
• Minimum 1 year of experience with disabled individuals, Nursing Home, or Home Care.

Responsibilities

• Assist with daily living skills and personal care.
• Promote community integration and social interaction.
• Implement outcomes from a Support Plan.
• Complete paperwork and documentation.
• Administer medications per state and agency requirements where applicable.
• Accompany residents to medical appointments as needed.
• Maintain living environments per regulations and health standards.
• Provide transportation to consumers as needed.
• Be available in emergencies and work with staff to support the consumer.
• Communicate with and update the Supported Living Coach regularly.
• Assist with recreational, cultural, and vacation planning as requested.
• Provide independent and functional living instructions.
• Complete all mandatory trainings required by APD.
