• Assist with activities of daily living (ADLs), including personal hygiene, bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications as trained and authorized.
• Assist individuals with mobility, transfers, and use of adaptive equipment as needed.
• Support individuals in developing and maintaining independent living skills.
• Accompany and transport individuals to appointments, recreational activities, community outings, and social events.
• Implement Individual Service Plans (ISPs) and document progress toward goals.
• Complete accurate and timely documentation, including daily notes, medication administration records, and incident reports.
• Maintain a safe, clean, and supportive environment.
• Monitor health and behavioral concerns and report changes to supervisors and appropriate team members.
• Promote dignity, choice, self-advocacy, and person-centered care.
• Follow all DODD, county board, agency, and state regulations and policies.
