Job summary

JobDiva, Inc. is seeking immediate Direct Support Professional (DSP) contract opportunities in Hawthorne, NY. You will provide direct care, support residents’ well‑being and safety, coordinate medical appointments and transportation, and advocate for appropriate care.

Requirements include a HS diploma or GED, valid NY driver's license with own transportation, OT flexibility, and completion of OPWDD certifications and SCIP/AMAP training.

Qualifications

Responsibilities

• Provide direct care and support to individuals, ensuring their well-being and safety.
• Coordinate and attend medical appointments, ensuring timely and safe transportation for residents.
• Advocate for residents during medical appointments and ensure appropriate follow-up.
• Document medical appointments, treatments, and medication administration, and communicate updates to staff.
• Support individuals in daily activities, including personal care, recreational programs, and household tasks.
• Develop and maintain positive relationships with residents, families, staff, and external providers.
• Ensure a clean, safe living environment following health regulations.
• Participate in interdisciplinary treatment meetings and contribute to treatment plans.
• Complete required training including SCIP and AMAP and apply knowledge daily.

Notes:

• Previous healthcare experience required.
• Experience working as a DSP, Social Worker, Recreational Therapist, Registered Behavioral Technician, Behavioral Specialist or a related position preferred.
• Must possess a HS Diploma/GED, valid driver's license and own transportation.
• Must be flexible to work OT as needed.
• Possible temp to perm opportunity.

Pay Rate: $22.00/hr

Work Schedule:

• 7:30am-3:30pm (Monday to Friday)
• 40 hours per week

• Accurately document medical appointments, treatments, and medication administration, communicating updates to relevant staff.
• Support individuals in their daily activities, including personal care, recreational programs, and household tasks.
• Develop and maintain positive, professional relationships with residents, their families, staff, and external healthcare providers.
• Ensure a clean, safe, and organized living environment, adhering to all health and safety regulations.
• Participate in interdisciplinary treatment meetings, contributing to the development and implementation of individualized treatment plans.
• Complete required training, including Strategies for Crisis Intervention Prevention (SCIP) and Medication Administration (AMAP), and apply this knowledge in your daily responsibilities.

Qualifications

• Must have valid Driver's License to drive in New York State.
• Must have a High School diploma or GED equivalent.
• One year experience with working with an OPWDD population is preferred.
• Must successfully complete OPWDD Medication Certification Course at the discretion of the Department Head, CPR, and First Aid course, and SCIP-R training.
