Job summary

Upstate Caring Partners is seeking a Direct Support Professional to implement individualized plans and support the daily needs of residents in our community-based programs. You will help with personal care, monitor health and safety, and document care provided.

As part of a team, you will transport participants to activities, assist with household tasks, and participate in agency events. Paid training is provided; full-time, part-time, and weekend shifts are available.

Qualifications

• Valid NYS Driver’s License required.
• No prior experience required; paid training provided.

Responsibilities

• Implement individualized plans and oversee health, safety and well-being of the people we support.
• Attend to personal care needs and maintain documentation in residential/day habilitation sites.
• Transport and accompany people on community activities and assist with daily housekeeping.

Job description

The Direct Support Professional (DSP) is responsible to implement each individualized plan, attend to personal care needs, oversee the health, safety and well-being of the people we support, keep nurses advised, participate in Agency activities, transport and accompany people we support on activities in the community, assist in daily housekeeping and other facility-related duties and maintain documentation in either or both the residential and/or day habilitation / community habilitation/ school age program site.
