Responsibilities

• Assist individuals with daily living activities, including personal hygiene, meal preparation, and medication administration
• Provide emotional support and companionship
• Assist with transportation to appointments and community activities
• Document and report any changes in individuals' behavior or health status
• Collaborate with team members to develop and implement individualized care plans

Requirements

• High school diploma or equivalent
• Experience working with individuals with mental health diagnoses preferred
• Ability to work independently and as part of a team
