Job summary

Caring Hands & More Llc in Iowa City is seeking a Direct Support Professional for the Day Habilitation program to help participants pursue personal goals, enjoy recreational activities, and engage with the community.

You will work closely with clients to support ISP goals, document care, and follow safety and infection control practices, while maintaining confidentiality and collaborating with a multidisciplinary team.

Qualifications

• Must be at least 18 years old with a high school diploma or equivalent.
• Experience in direct support services or caregiver role preferred.
• Able to read, write and follow a written Individual Plan of Care.
• Effective verbal, written, and interpersonal skills.
• Basic to intermediate computer skills.
• Possess and maintain a valid driver’s license, auto insurance, and reliable vehicle.

Responsibilities

• Interacts with clients to support the completion of daily goals as described in each client’s ISP.
• Provides support to allow participation in recreational activities and community involvement.
• Assists in the administration of medications ordinarily self-administered per company policy.
• Monitors and reports changes in the client’s condition to the supervisor.
• Documents care provided in the client’s records.
• Promotes personal safety and infection control practices.
• Communicates with the interdisciplinary team during meetings and reports.
• Maintains confidentiality in all aspects of the position.
• Attends trainings to meet compliance requirements.
• Performs other related duties as deemed necessary.

POSITION SUMMARYThe Direct Support Professional for the Day Habilitation program is responsible for assistance and support to participants’ individual goals, recreational activities, and community involvement while maintaining the individual’s personal comfort.ESSENTIAL DUTIES AND RESPONSIBILITIESInteracts with the clients to support the completion of daily goals as described in each client’s Individual Service Plan (ISP).Provides the necessary support to allow individuals to participate in recreational activities and facilitate community involvement.Assist in the administration of medications that are ordinarily self-administered according to company policy.Monitors and reports any observed changes in the client's condition and/or needs to the supervisor.Documents the care provided by entering the required information into the client's records.Promotes personal safety and maintains a safe environment for clients by observing infection control practices.Effectively communicates with all members of the interdisciplinary team through verbal reports and staff meetings.Maintains confidentiality in all aspects of the position.Attend in-service programs and trainings to meet compliance requirements.Performs other related duties and responsibilities as deemed necessary.PERKS & BENEFITSFull-time employees are eligible for the following benefits:Medical, Dental, and Vision CoverageDisability InsuranceSimply IRA with company matchingPaid Leave TimeHoliday PayCasual Work EnvironmentQUALIFICATIONSMust be at least 18 years of age and possess a minimum of a high school diploma or equivalency.
