• Excellent verbal and written communication with members and families.
• Able to work effectively in a team and follow directions.

Responsibilities

• Cultivate a safe, supportive relationship with members and understand service plans.
• Provide individualized support to promote independence and community involvement.
• Assist participants in developing and following a meaningful activity schedule.
• Follow Service Implementation Plans and care plans per Person-Centered Plans.
• Maintain accurate daily documentation of services and progress.
