Job summary

Community Mainstreaming is seeking a Direct Support Professional to help individuals with developmental disabilities live fulfilling, independent lives. You’ll assist with daily living skills, personal care, transportation, and community participation while ensuring health and safety and recording progress.

This role offers meaningful work, a supportive team, and opportunities for growth. Evening and weekend shifts are available with $20.50/hr and comprehensive benefits for eligible staff.

Qualifications

• High school diploma or GED is required.
• Valid NYS driver license with 1+ year driving experience.
• Experience as caregiver, direct care worker, CNA, HHA, or companion is advantageous.
• Preferred experience with intellectual and developmental disabilities.
• Ability to operate agency vehicles and lift up to 50 lbs.
• Evening and weekend shifts are preferred.
• Smartphone is required.

Responsibilities

• Support individuals to reach personal goals from daily tasks to achievements.
• Provide transportation to activities and medical appointments to keep clients connected.
• Support behavioral modifications and communication improvements.
• Develop service plans with team to help individuals reach life goals.
• Promote health and safety of individuals.
• Maintain documentation of service activities and daily notes.

Minimum Qualifications

• High school diploma or GED required.
• Valid NYS Driver License with at least 1 year of driving experience.
• Previous experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Health Aide, or Companion is advantageous.
• Preferred experience working with individuals with intellectual and developmental disabilities.
• Ability to operate agency vehicles.
• Capability to lift up to 50 lbs.
• Evening and weekend shifts are preferred.
• Smartphone required.

Paid Time Off

• Up to 80 hours of vacation during the first calendar year of employment. Employees accrue an additional 8 vacation hours each subsequent calendar year, up to a maximum of 120 hours.
• Up to 40 hours of personal time per calendar year.
• 10 paid holidays.

Pay

$20.50/hr.

What You Will Do

• Support individuals to reach their personal goals, from daily tasks to personal achievements.
• Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community.
• Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives.
• Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals.
• Support and promote the individual’s health and safety.
• Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.
• Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping.

Work Hours/Schedule

Full‑time, Part Time, and Evening and Overnight ONLY. (2 weekend short sifts 12 hours)
