Job summary

Sunrise Community, Inc. is seeking a dedicated support worker to provide supervision and assistance to individuals with intellectual and developmental disabilities to help them live valued lives in the community.

You will support and implement plans, monitor health and safety, report concerns, and promote independence and choice in daily activities while collaborating with a multidisciplinary team. Qualifications include a valid driver’s license with endorsements, background and drug screening,

Qualifications

• Must possess a valid Driver's License with endorsements and have 3 years of driving experience.
• Must possess basic computer skills.
• High School Diploma or equivalent if required by work location.
• Minimum one year of experience in human services if required by work location.

Responsibilities

• Identifies and reports any suspected abuse or violations of an individual’s rights.
• Provides support, assistance and supervision to meet the needs and goals of the individuals according to their plans.
• Ensures that each individual’s training, behavior and support programs are implemented and documented per regulatory requirements.
• Ensures health, safety and well‑being of individuals at all times.
• Provides a positive environment that promotes independence and choice.
• Follows positioning schedules and uses equipment per plan.
• Assists with appropriate diet, texture and equipment.
• Keeps homes, outside areas and vehicles well maintained and organized.
• Performs all other duties as assigned.

Essential Functions

• Provides necessary level of assistance for appropriate diet, texture and equipment.
• Ensures all homes, outside areas and vehicles are well maintained and organized.
• Performs all other duties as assigned.
